	Register With The Practice?

A person may request to be considered for application to join the practice list by completing the practice request form.

Having fulfilled the criteria to apply to join the list you will be given a new patient questionnaire to complete. You should return the completed questionnaire along with your previous NHS medical card. If you have lost your medical card you will need to fill in a GMS1 form which is available from reception.

You will be registered as a patient of the GP partnership. However, you will be able to see any of the doctors of your choice dependent upon whether there is an available appointment for that doctor. We cannot guarantee you can always see a female doctor.
